model questionnaire / agency questionnaire







In order for Arts C Models to best evaluate candidates for modelling contracts, I have compiled a short list of essential material that we need to process beforehand.

Please note: ONLY models that are represented by an agency will be eligible for a full sponsorship contract (ie. Impact Division). Those who do not have agency representation will be considered for our Team Contract and Working Visa contract.

Also, we are NOT offering any contracts to male models at the moment.

All documents whether it be photos, answers to our questionnaire, composite cards, whatever material sent to Arts C Models or myself will be held in the strictest of confidence and WILL NOT be shared with any other party NOR returned to the sender. Although we do our very best to answer everyone's application, successful candidates will be contacted in priority.

Fair enough.  With that said on with the steps!





If you are a model, with agency representation or not and you are interested in modelling with Arts C Models in Tokyo, Japan  here's what we need you to do:


  1. Fill out our model questionnaire.

  2. ALL candidates MUST then send copies of your portfolio via post to our agency here in Japan to be considered. Please include as many photos and/or copies as you can in this package; for example...

    - various poses and outfits (smiling, serious, cool, whatever!)

    - head shots (smile and no smile)

    - profile and semi-profile shots (45' and 90')

    - full body shots (head to toe) with a bathing suit and with tight fitted clothing.

    - copies of any jobs you might have done. (tear sheets)

    If you have access to a scanner, you may ALSO send NO MORE than 4 photos including a head shot and a full length shot of yourself in the form of an email to:

    **(Please remember that ONLY those candidates that send photos to our agency via air mail will be considered.

    Also photos sent by email should be either zipped or extremely small in file size!! (I cannot stress this enough)***
    (MAXIMUM TOTAL OF 500 KB!)

    And that's all we need to start things off!  Atsushi or I will then contact you with feedback as soon as possible.





    If you represent an agency that is interested in sending models to work with Arts C Models in Tokyo, Japan  here's what we need you to do:


    1. Fill out our agency questionnaire.

    2. Please send to Arts C Models c/o Atsushi Miyakoshi: (see bottom for the address)

      (i) your agency head sheet or book.
         (if available)

      (ii) composite cards of models you believe may have great potential to work in Japan.

    And that's all we need to get things going.  Atsushi or I will contact you with the models that we are interested in and request further details.
